A short time ago, we launched our Concierge Care Model. We are excited to share the overwhelming positive response we have received from the community regarding this model!
We can't begin to tell you how pleased our patients and their families are when they receive their monthly inner balance and health management call from our licensed clinicians!
This Concierge Care Model was built as a world class service aimed at providing overall healthier outcomes and increased communication for our patients! As part of the continuity of care service we provide, our goal is to ensure our medical team is engaged with our patients' health at all times.
What's included:
To ensure we provide the most comprehensive approach to our patients healthcare plan, we partner with our local hospitals, nursing homes, rehabilitation centers and other facilities to deliver your ongoing medical care, wherever they are. This ensures the same medical team is connected and engaged with our patients family, health history, prescription background and your personal story.
Monthly care plan review call with patient and/or health care surrogate; the plan will help support the patient’s disease control and health management goals, including physical, mental, cognitive, psychosocial, functional and environmental factors. We will also keep track of referrals and community services.
Patients and their caregivers will gain a team of dedicated health care professionals who will help them plan for better health and stay on track
Transition of Care Assistance
For those patients classified as homebound, no worries! We travel to our patients. We make house calls in the Bradenton and Sarasota communities.
In addition to our providers rounding at facilities, we also have a nurse that rounds monthly at our partnered facilities to reconcile patient charts and to review overall patient physical and mental condition.
ALL OF THIS - AT NO ADDITIONAL COST TO THE PATIENT. Between me and you, I think this is the reason why our Concierge Care model is loved so much! :)
